We hereby request the opening of an account in our name. The application process has been completed, and we agree to settle all payments related to the account referenced below, in accordance with the terms discussed with your representative and by the due date specified on each invoice.

Furthermore, we, the applicant, confirm that we have read and fully agree to the Conditions of Trading Act Disclosure and Consent as outlined at the bottom of this page.
